After drug was administered, patient experienced the following side effects: cardio-respiratory arrest. Tizanidine dosage: unknown. During the same period patient was treated with OXYCODONE, IBUPROFEN, ACETAMINOPHEN, CARISOPRODOL, PROMETHAZINE. Patient died.
